More Than a Baby Shower: How Coordinated Care and Compassion Are Changing Lives

At Common Good Community, we believe in celebrating life and removing barriers. This past Saturday, those values came to life as we hosted a Community Baby Shower designed not just to give—but to connect, empower, and support.

Thanks to the generous partnership of Savannah R. and Anthem Blue Cross Blue Shield-Medicaid Health Plans, along with a powerful coalition of community organizations, we welcomed 12 amazing moms and 34 total attendees into a space of joy, care, and dignity.

Essentials and Beyond

Each family left with more than gifts. Diapers, wipes, baby carriers, hygiene kits, eating kits, and books were thoughtfully prepared to meet their needs—and five families received cribs to ensure their babies have a safe place to sleep.

But this event wasn’t only about what was given—it was about how families were seen, supported, and served. Through shared moments, warm conversations, and compassionate care, we built trust that extends well beyond a single day.

Tech with a Human Touch

Innovation was key to making the event seamless and supportive. Our integrated tech system made sure no one had to retell their story at every step—and allowed our Coordinated Care Companions (CHWs) to do what they do best: walk alongside families with empathy and expertise.

  • QR codes made on-site check-in fast and friendly.

  • Pear Suite allowed our CHWs to provide real-time intakes, Medi-Cal eligibility checks, referrals, and documentation—all during the event.

The result? A process that was:

  • Seamless and trauma-informed

  • Efficient and non-repetitive

  • Empowering, with visual care progressions that made every mom feel seen and supported

Real Outcomes, Real Impact

  • 5–10 families enrolled in a medically tailored meal program

  • 2 families received health screenings and education

  • 1 family has already completed a follow-up care appointment

This is what coordinated care looks like when technology meets humanity—when health equity isn’t a buzzword but a baseline.
