CalAIM Initiative: ECM and CHW Services
Through California’s CalAIM initiative, Common Good Community offers two key supports: Enhanced Care Management (ECM) and Community Health Workers (CHWs). Together, they make sure Medi-Cal members receive whole-person care that looks at both health and daily life.
This means helping people navigate the healthcare system, connecting them with resources like food, housing, and transportation, and checking in regularly to ensure ongoing support. By bridging medical care with real-life needs, CHWs and ECM give individuals and families the tools to live healthier, more stable lives.

Who Qualifies for CHW & ECM Services?
These services are available to Medi-Cal members with complex health and social needs, including:
Individuals experiencing homelessness
Adults & children with serious mental illness or substance use disorders
Frequent users of emergency services
People with multiple or chronic conditions
Individuals transitioning from incarceration or institutional settings
At Common Good Community, we focus on supporting Medi-Cal members who are most at risk. This includes adults, children, and youth who often end up in the emergency room or hospital when it could be prevented; individuals who are pregnant or postpartum and need extra support for themselves and their babies; adults and youth returning to the community after incarceration or time in a correctional facility; and families working toward birth equity and healthier outcomes.

No Wrong Door Access
At Common Good Community, we believe support should be available whenever people need it—not just during business hours or through a single entry point. That’s why we follow a “No Wrong Door” approach, ensuring individuals can connect to care in the way that works best for them, whether in a moment of crisis, during a transition, or when facing everyday challenges.
To expand this accessibility, our Coordinated Care Companion app (coming soon) will make it even easier to find help. Through the app, individuals will be able to search resource directories like 211 and Find Help, call a Coordinated Care Companion directly for immediate support, submit an Assistance Request or Appointment Form at any time, and discover resources tailored to their unique needs—all from one simple platform.
