What We Do
FREE Community Health Services and Coordination
Through California's CalAIM initiative, Common Good Community offers two key supports: Enhanced Care Management (ECM) and Community Health Workers (CHWs). Together, they ensure Medi-Cal members receive whole-person care—connecting you with food, housing, and transportation resources, helping you navigate the healthcare system, and checking in regularly so support does not stop after one visit.
What You Can Expect From Our Team
Personalized care plans built around your needs
Coordination across your providers and commitments
Support addressing housing, food, and other life needs
Regular check-ins so you are never left without guidance
Who Qualifies for CHW & ECM Services?
These services are available to Medi-Cal members with complex health and social needs, including:
Individuals experiencing homelessness
Adults and children with serious mental illness or substance use disorders
Frequent users of emergency services
People with multiple or chronic conditions
Individuals transitioning from incarceration or institutional settings
Individuals who are pregnant or postpartum and need extra support
Adults and youth returning to the community after incarceration
Families working toward birth equity and healthier outcomes
At Common Good Community, we focus on supporting members who are most at risk—including those who frequently end up in the emergency room or hospital when it could be prevented.
For a deeper look at how Enhanced Care Management supports members across California, you can explore the ECM Member Toolkit (PDF) created by the Department of Health Care Services.
